This evaluation assesses the effectiveness and impact of the Cardiovascular Disease (CVD) Prevention Programme implemented in Bristol, North Somerset and South Gloucestershire. The programme was conducted by Accure Healthcare Consultancy Ltd (Accure). This programme was designed following an initial hypertension case finding pilot in Inner City and East Bristol (ICE), which was also conducted by Accure.
The final programme was designed to raise awareness and address key risk factors associated with CVD, including hypertension, high cholesterol, diabetes and Atrial Fibrillation (AF). It then provided tailored health advice to individuals to reduce their risk of CVD. This was done through targeted interventions such as health education, lifestyle modification support, and, where appropriate, referral to their General Practice for further monitoring and/or treatment.
