A propos de nous

The Inner City and East Bristol Locality Partnership is one of six locality partnerships across Bristol, North Somerset and South Gloucestershire, serving 134,800 people across 12 GP practices.

Our Board brings together local community organisations to work collectively to improve care and plan services. Using population data, evidence and community engagement, the Board helps coordinate and support new integrated models of care.

The Board does not deliver services but works with community partners to facilitate improvements and join up work where needed.

Our priorities

Our key priorities are to:

  • Reduce inequalities in mental health experiences and outcomes amongst marginalised communities
  • Develop biopsychosocial interventions to enhance the health and wellbeing of the local population
  • Use local and national data to inform our approach to reducing the high prevalence of severe frailty in people aged 75 and over, from Asian, mixed and Black communities
  • Decrease the prevalence of excess weight in school-aged children, from reception to year 6
  • Understand the barriers and inequalities in accessing mental health services for school-aged children up to 18 years old and identify solutions to improve access and experience
  • Reduce preventable falls and harm through strategic coordination across organisations
  • Reverse the pattern of early ageing by focusing on prevention and better management of contributors to cardiovascular disease, particularly diabetes, hypertension, inactivity and weight.
