Contents
We deliver a number of projects to improve health, wellbeing and access to care in our communities. Some of these are listed below.
Inner City and East Bristol continues to support Green Social Prescribing initiatives for our local community.
In 2024/25 we funded seven organisations to deliver nature-based activities, such as gardening, Nordic walking, and wild swimming, aimed at improving mental health, physical activity and social connection.
The activities were targeted at our under-served communities, in a bid to reduce GP demand, support recovery and tackle isolation.

Workers offer personalised care and translation, using trauma-informed approaches, to improve engagement and reduce inequality.

Using the health inequalities funding Inner City and East Bristol Locality Partnership supports the following projects:
Black Mother’s Matter – an organisation working to improve outcomes for expectant mothers racialised as Black. They provide support to health professionals to help them better support Black mothers to give birth, by providing resources, tailored guidance and signposting to further support.
Caafi Health Community Clinics – delivering accessible health checks, advice, and long-term condition management in community settings, in multiple languages. This helps support early detection of conditions such as diabetes, hypertension and cancer, linking residents with GPs and culturally appropriate prevention advice.
Caafi Health Fruit and Vegetables on Prescription – this project focuses on supporting local families in Inner City and East Bristol who are at risk of food inequality, by providing them with weekly fruit and vegetables. The project targets families living in poverty, those on benefits, those with long term health conditions or those from ethnic minority groups.
Dementia Support Project – this is a partnership between the Chinese Community Wellbeing Society, Bristol Black Carers, and Dhek Bhal. The project delivers dementia screening, healthy ageing workshops, carer peer support and targeted interventions for ethnic minority communities.
Nature Consortia – a collaboration between Open Mind’s Active, ActiveBeing and Soul Trail to support women’s mental and physical wellbeing, through a range of nature-based interventions.
Drop in and Reach out – a programme led by SARSAS that offers fortnightly safe, supportive drop-in spaces from a central Bristol venue and in Inner City and East Bristol community venues for survivors of sexual violence and abuse.
Gardening for Health – a project led by the Wellspring Settlement to support people to engage in nature-based interventions and activities to improve their physical and mental health, by building links with partner organisations and supporting people to connect through activities, groups, and services that support health and wellbeing.
St Werburgh’s City Farm – this project delivers a volunteering, skills and wellbeing programme of practical courses, placements and activities for people experiencing mental health challenges and/or exclusion and marginalisation from the workplace.
