A propos de nous

The South Gloucestershire Locality Partnership is one of six locality partnerships across Bristol, North Somerset and South Gloucestershire, serving 290,400 people across 22 GP practices.

The South Gloucestershire Locality Partnership brings together Voluntary, Community and Social Enterprise (VCSE) and health and social care organisations within South Gloucestershire.

We aim to develop new, integrated approaches to delivering wellbeing, health and care, working collaboratively to benefit individuals and communities across South Gloucestershire.

Our priorities

Working with our communities, and drawing on our resources and expertise, we support local people to reach their full potential. Our priorities are to:

  • Improve mental health and wellbeing for children and young people, including addressing self-harm, emotional-based school absence and anxiety, through integrated working with partners
  • Improve the outcomes of those experiencing chronic pain, depression and/or anxiety in our communities by providing holistic, de-medicalised intervention and peer support
  • Explore insight and data on pain and Musculo-skeletal (MSK) conditions and scope further work 
  • Support individuals to be active within their communities and maintain their independence as they age
  • Recognise and support those with caring responsibilities and address isolation and loneliness in our rural locations
  • Deliver improved outcomes for individuals at risk of falling through a preventative and proactive approach
  • Learn from local models that are being trialled in the community to improve outcomes for people who are frail with complex needs
  • Grow our Asset-based Community Development (ABCD) approach by developing and implementing the South Gloucestershire ABCD Framework, supported by Community Inclusion Grants
  • Explore the Better Care Fund as a mechanism to support further joint working and a shift towards more personalised and proactive care.
