A propos de nous

Woodspring Locality Partnership is one of six locality partnerships across Bristol, North Somerset, and South Gloucestershire, serving 131,100 people across six GP practices.

Woodspring covers the areas of Portishead, Clevedon, Nailsea and Pill and has the highest rate of frailty across Bristol, North Somerset, and South Gloucestershire, with over 20% of the population aged 65 and over.

The Partnership brings together health, care, the voluntary sector and community, strengthening the trusted relationships that support Clevedon, Portishead, Pill, Nailsea and our rural communities. By combining local insight, population health data and the strengths of each partner, we design and deliver joined‑up, place‑based care that responds to people’s needs and supports healthier, more connected communities.

Our priorities

Our key priorities are to:

  • Focus on population health and wellbeing
  • Work with communities and the voluntary sector to build on and nurture existing skills, knowledge and connections
  • Join up care in the community by delivering preventive and proactive care
  • Make the community the default setting for care, meeting the majority of people’s needs close to where they live
  • Engage with local communities in co-production
  • Optimise our resources to deliver efficient and effective services.
