Nous avons port\u00e9 sur ces soins, c&#039;est-\u00e0-dire le soutien apport\u00e9 aux patients lorsque leur \u00e9tat de sant\u00e9 se d\u00e9t\u00e9riore et qu&#039;ils risquent d&#039;\u00eatre hospitalis\u00e9s. Parmi les participants figuraient des cliniciens, des patients, des aidants, des membres du public, des m\u00e9decins g\u00e9n\u00e9ralistes, des associations, des prestataires de services communautaires, des collectivit\u00e9s locales, des partenaires en mati\u00e8re de d\u00e9veloppement durable et de transformation, et bien d&#039;autres. Le r\u00e9sum\u00e9 pr\u00e9sent\u00e9 ici met en lumi\u00e8re les principaux th\u00e8mes abord\u00e9s lors des \u00e9changes.
