We have put new resources into mental health and primary care, provided good practice guidance and set up a project in Inner-City Bristol to ensure that particular ethnic minority communities are treated fairly.<\/p>\n<p>\u201cGoing forward we will be focusing our attention on ensuring that the health checks are followed up, that we support the engagement of those small numbers of people who continue not to receive any of the health checks and that we take specific action to address the particular health inequalities faced by some ethnic minority communities.\u201d<\/p><\/blockquote>\n<p>Anyone aged 18 or over who has schizophrenia, bipolar disorder or psychosis is eligible for a free physical health check once a year.<\/p>\n<p>Primary Care Network, 4PCN, in South Gloucestershire is one set of practices that have developed strong processes for engaging their patients with severe mental illness and following up the physical health checks.<\/p>\n<p>Sarah Cole, Primary Care Network Manager for 4PCN, which is made up of Hanham Health, Kingswood Health Centre, Cadbury Heath Healthcare and Close Farm Surgery, explained:<\/p>\n<blockquote><p>\u201cWe recognised that patients with severe mental illness face additional challenges, and we wanted to break down the barriers to accessing care and support for them.<\/p>\n<p>\u201cWe have a dedicated team of Care Coordinators who not only do the physical health checks in a location where the patient feels comfortable, such as their own home, but they also take time to listen to what the patient and their carers might need in order to look after their health and support them to achieve those goals.<\/p>\n<p>\u201cWe have seen some fantastic examples of where this has made a tangible difference to the health and wellbeing of our patients and are very proud of our team.\u201d<\/p><\/blockquote>\n  <\/div>\r\n  <\/section><\/div><!-- .assembler_text-module.assembler_module.assembler_text-module_1.assembler_module_1..transparent --><\/div><!-- .assembler_module_area.assembler_module_area_1 --><\/div><!-- .assembler_default-group.assembler_module_group.assembler_default-group_1.assembler_module_group_1  -->","protected":false},"excerpt":{"rendered":"<p>L&#039;organisation du NHS charg\u00e9e de la planification des services de sant\u00e9 \u00e0 Bristol, North Somerset et South Gloucestershire (BNSSG) a d\u00e9pass\u00e9 la norme nationale pour ses examens de sant\u00e9 physique des personnes atteintes de maladie mentale grave (SMI) et figure d\u00e9sormais parmi les zones les plus performantes du pays.  <\/p>","protected":false},"author":24,"featured_media":0,"comment_status":"closed","ping_status":"closed","sticky":false,"template":"","format":"standard","meta":{"_acf_changed":false,"footnotes":""},"categories":[23,301,1],"tags":[96,842,843],"class_list":["post-25647","post","type-post","status-publish","format-standard","hentry","category-health-services","category-mental-health-services","category-uncategorized","tag-mental-health","tag-physical-health-checks","tag-severe-mental-illness"],"acf":[],"_links":{"self":[{"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/posts\/25647","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/posts"}],"about":[{"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/types\/post"}],"author":[{"embeddable":true,"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/users\/24"}],"replies":[{"embeddable":true,"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/comments?post=25647"}],"version-history":[{"count":0,"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/posts\/25647\/revisions"}],"wp:attachment":[{"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/media?parent=25647"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/categories?post=25647"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/bnssghealthiertogether.org.uk\/fr\/wp-json\/wp\/v2\/tags?post=25647"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
