Innovate Healthier Together team facilitates stakeholder workshop hosted by the Integrated Care @ Home Board<\/h1>\n

The Innovate Healthier Together team were invited to facilitate a stakeholder workshop hosted by the Integrated Care @ Home Board Workshop on 25 April. The event was attended by system partners and healthcare organisations including One Care, Sirona, Local Authority Public Health, BrisDoc, Healthwatch, Age UK, Community Pharmacy Avon, GPs and NHS Bristol, North Somerset and South Gloucestershire ICB.<\/p>\n

The Integrated Care @ Home Board is developing a model for community-based care that improves outcomes and efficiency for our communities.\u00a0 The focus of this model is not on specific pathways or conditions, but on how we can better support the most complex patients in our system, acknowledging that if we can address this, then the benefits will be seen across our whole population.\u00a0 The focus of the session was to define a problem statement and review the next steps to support the development of a model for integrated community service provision.<\/p>\n

Following an extreme game of rock, paper, scissors, the energised group were introduced to design-thinking techniques which were structured around the Design Council’s Double-Diamond design and innovation process. The group used the West of England Academy\u2019s creative problem-solving toolkit<\/a>, specifically focussing on clarifying the challenge and identifying the issues. The session guided the workshop participants through exercises that helped them unpick and better understand the current challenges in integrated care for patients who access many services, and then rapidly brainstorm ideas to identify opportunities to address them.<\/p>\n

The Integrated Care @ Home Board has committed to taking the outputs of this session and using them to inform service redesign recommendations to be implemented in the summer of 2024.<\/p>\n <\/div>\r\n <\/section><\/div><\/div><\/div>","protected":false},"excerpt":{"rendered":"
