We deliver a number of projects to improve health, wellbeing and access to care in our communities. Some of these are listed below.
Building strong relationships with community anchor organisations in South Bristol helps us hear directly from local people about how our health priorities affect them and understand the wider inequalities they face.
This local insight is informing our approach to how we collectively tackle health inequalities to improve the health and wellbeing of local people. The insights shared as part of this network feed into the design and delivery of projects led by South Bristol Locality Partnership.

The Wellbeing Network consists of 24 organisations that work together to support children and families in South Bristol to access fresh foods and lead more active and healthier lives. The network’s focus has broadened from school-age children to the wider community, promoting early intervention, consistent nutrition guidance and healthier environments.
By strengthening communication with families, linking with hospital programmes, and equipping practitioners, the group aims to reduce health inequalities and empower communities to build lasting healthy habits.
The development of Community Connectors builds on the need for support for growing numbers of individuals who have become increasingly visible because of the COVID-19 pandemic.
The reliance and expectation that everyone can access everything they need online, or via an app, is unrealistic, and particularly affects older and financially vulnerable people in South Bristol.
Community Connectors provide a person for individuals to talk to in their community to help them access local resources, to improve wellbeing and to prevent their health deteriorating.
These roles support people experiencing isolation and loneliness in BS3 and Knowle West to help them build social connections, remove barriers to services and improve independence and wellbeing.

We are working together with partners across the voluntary and community sector, health and care, local authority and public health to prevent the rise of prediabetes in South Bristol. As of 2024, South Bristol has more than 9,000 patients registered as Type 2 Diabetic, and over 11,000 have been diagnosed as prediabetic, which means they have an increased risk of developing Type 2 Diabetes if no lifestyle changes are made.
Through local investment we aim to prevent rising diabetes rates through working with partners to deliver a targeted community-led health intervention. We work with statutory and community partners, and people with lived experience to prevent the rise of prediabetes and the risks of developing further long-term conditions in South Bristol.
Our Community Innovation Lead role forms part of our local response to the Fuller report. The role supports an ambitious and joined-up approach across local voluntary and community organisations and health providers to increase collaboration and champion prevention.
The role aims to improve service coordination across local voluntary and community organisations and health providers, share best practice and enable targeted health and wellbeing interventions to reach people that need them most.
