NHS continuing healthcare (CHC) is a package of care arranged and funded solely by the NHS for individuals who are not in hospital, but who have on-going complex, intense, or unpredictable healthcare needs. You can receive CHC services in any setting, including your own home or a care home.<\/p>\n
If you are eligible for CHC, Bristol, North Somerset and South Gloucestershire Integrated Care Board will fund the care required to meet your identified care needs.<\/p>\n
