Living Well with COPD
Living Well with COPD is a pilot programme that has been put in place in Bristol, North Somerset and South Gloucestershire to better support people with chronic obstructive pulmonary disease (COPD) at home and help reduce unnecessary admission into hospital.
It provides people with access to a specialist My mHealth smartphone app, MyCOPD, to help manage their condition at home. A small proportion of people will also be given access to remote monitoring equipment from Doccla to enable them to be monitored by digital health advisors or specialist community health and care staff who will be able to intervene with at-home support, if required.
People who also have a cardiovascular condition, will be offered access to the myHeart app to use alongside their myCOPD app, using one single log in.
Local people with COPD are being contacted directly with an invite to access the support from Living Well with COPD.
What are the benefits of Living Well with COPD?
Many people with COPD spend time at hospital if their condition deteriorates, particularly during the colder winter months. With Living Well with COPD, people will be supported and empowered to take better control of their health through improved education on their condition and how best to manage it. This support will keep more people well at home and out of hospital, where we know people sleep better and have a more positive outlook, with home comforts and family, friends or carers nearby.
