Personalised Medication Support Service

The Personalised Medication Support Service is for anyone who finds it difficult to take medication on a regular basis. This can be for yourself, a family member, or someone you are caring for.

As people age, they can develop conditions like diabetes or high blood pressure. It’s important to take medications regularly to manage these conditions. We understand that when people are taking many different medications and have problems remembering what to take and when, this can be difficult.

The Personalised Medication Support Service can help by providing support through the use of electronic reminders. These could be simple text messages on your phone, or internet enabled video or audio base reminders. Our community project workers are trained to assess your needs and recommend what system might be best for you.

What help is available?

If you, or someone you are caring for is having difficulty remembering to take their medication on a regular basis, there are resources available to help.

If it is difficulty because of issues with swallowing, dexterity or vision they can visit their GP or Local Pharmacy, for advice. The Council Technology Enabled Care Team do provide a range of basic devices that can help. These could be items such as large print labels, pill splitters or soluble options for medication.

If the issues are due to memory or understanding, they may benefit from support using technology, depending on their situation. Below are some options:

  • SMS messaging reminders- this can be set up by your local Pharmacy.
  • Medisafe Pill Reminder App- This is a free Smartphone app which can be set up to provide reminders through the day
  • YourMeds Dosette Box- The digital display shows the user which box to open and alerts a carer if a dose has been missed.
  • My Home Helper- This provides text reminders and allows video calls from a carer.
  • Sentai AI companion- helps with your daily routine, by providing audio reminders and messages.
