The service is delivered by Alzheimer\u2019s Society and Devon Partnership NHS Trust working together as the Bristol Dementia Partnership.<\/p>\n<p>People living with dementia will be referred to the Dementia Wellbeing Service through their GP.<\/p>\n<p>The Dementia Wellbeing Service has been designed to provide a personalised package of care, tailored to the individual. It includes continuous, one-to-one support local to where someone lives and the creation of a personalised wellbeing plan. This will provide long term support from assessment and diagnosis until the end of their life, including extensive support for those with the most complex and challenging needs. Families and carers will also benefit from practical help, training and advice.<\/p>\n<h4>Bristol\u2019s memory caf\u00e9s<\/h4>\n<p>Alzheimer\u2019s Society runs monthly memory caf\u00e9s at venues across the city for anyone who has dementia or memory problems. They also cater to those who have no formal diagnosis but are experiencing issues with their memory.<\/p>\n      <a href=\"https:\/\/www.thefiloproject.co.uk\/\" class=\"btn-link flex w-[fit-content] \" rel=\"noopener\">Visit The Filo Project website for further information or to make a referral<\/a>\n\n<h3>South Gloucestershire<\/h3>\n<p>We work with South Gloucestershire Council, <a href=\"https:\/\/www.wellaware.org.uk\/info-resources\/dementia\/\" target=\"_blank\" rel=\"nofollow noopener\">Well Aware<\/a> and other partners to ensure that people with dementia are supported.<\/p>\n<h4>Memory Cafes in South Gloucestershire<\/h4>\n<p>Memory Caf\u00e9s offer a friendly welcome for anyone affected by dementia, or worried about their or someone else\u2019s memory. They provide practical information and support, as well as the opportunity for people to ask questions and to listen to others\u2019 experience.
