Template: template-text - Matched: _only --><section class=\"w-full mx-auto max-w-content pt-0 sm:pt-0 pb-3 sm:pb-6\">\r\n  <div class=\"entry-content w-full !px-0 max-w-small\">\r\n        <p><span data-contrast=\"auto\">Our Population Health and Strategic Commissioning Plan\u00a0sets out the priorities\u00a0for\u00a0health and\u00a0social\u00a0care services\u00a0across Bristol, North\u00a0Somerset\u00a0and South Gloucestershire.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The creation of plans like this is a national requirement for all areas, and we have developed ours in close collaboration with <a href=\"https:\/\/www.nhsglos.nhs.uk\/\" rel=\"noopener\">NHS Gloucestershire Integrated Care Board<\/a> as we work to bring our organisations together as part of the\u00a0<\/span><a href=\"https:\/\/bnssghealthiertogether.org.uk\/changes-to-the-nhs-in-bristol-north-somerset-and-south-gloucestershire\/\"><span data-contrast=\"none\">NHS Changes<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">We have\u00a0three shared ambitions\u00a0\u2013\u00a0healthy lives, health\u00a0equity\u00a0and best value. Our focus\u00a0is\u00a0on year one, with the intention of having a single plan from 2027.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">This plan builds on everything we have done so far and helps us concentrate on our new role as a strategic commissioning organisation. Focusing on improving experiences strengthens outcomes and decreases avoidable costs.
