One Weston Bak\u0131m Evi Merkezi ulusal \u00f6d\u00fcle aday g\u00f6sterildi<\/strong><\/h1>\n

A local care home initiative, the first of its kind in the UK, has been nominated for a national Health Service Journal (HSJ) Award<\/a> for its support for care home residents in Weston-super-Mare.<\/p>\n

The One Weston Care Home Hub was created by the Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB), Weston Locality Partnership and the Pier Health Primary Care Network (PCN) and has been shortlisted for the HSJ\u2019s Place-based Partnership & Integrated Working Care Award.<\/p>\n

The service supports care homes in the area to tackle the inequalities experienced by older people, people with learning disabilities and mental health issues in Weston, through collaborative working and supporting care homes with reactive and proactive care delivery.<\/p>\n

Since September 2021, early successes of the One Weston Care Home Hub have shown a 45% reduction in emergency hospital admissions, 20% reduction in Emergency Department attendances and an 11% reduction in ambulance callouts to care homes.<\/p>\n

It has also boosted job satisfaction among care home staff and has hosted new training posts and leadership development for staff.<\/p>\n

Dr Holly Paris, Service Director for the One Weston Care Home Hub, said:<\/p>\n

“Care Homes have been through the worst of times during the pandemic. It\u2019s been a privilege to work with our care colleagues, their residents, and families during this challenging period to level up and create a legacy for some of our community’s most vulnerable adults.\u201d<\/p><\/blockquote>\n

David Moss, BNSSG ICB Head of Locality for One Weston, said:<\/p>\n

“In 20 years of working in the NHS I have not seen such an integrated, passionate group of staff, continually learning and developing by sharing cross sector skills and knowledge.<\/p>\n

\u201cThe permission and high levels of trust allow the team to respond fully to the need presented to them first hand. This is testament to progressive and bold clinical leadership.”<\/p><\/blockquote>\n

The HSJ Awards celebrates healthcare service excellence in the UK, highlighting best practice, improving patient outcomes and promotes innovation that drives improvements to better care.<\/p>\n

Winners of the HSJ Awards will be announced at a ceremony in London on Thursday 16 November.<\/p>\n <\/div>\r\n
