Etkinli\u011fe One Care, Sirona, Yerel Y\u00f6netim Halk Sa\u011fl\u0131\u011f\u0131, BrisDoc, Healthwatch, Age UK, Community Pharmacy Avon, GP'ler ve NHS Bristol, North Somerset ve South Gloucestershire ICB dahil olmak \u00fczere sistem ortaklar\u0131 ve sa\u011fl\u0131k kurulu\u015flar\u0131 kat\u0131ld\u0131.<\/p>\n<p>Integrated Care @ Home Kurulu, toplumlar\u0131m\u0131z i\u00e7in sonu\u00e7lar\u0131 ve verimlili\u011fi art\u0131ran toplum temelli bak\u0131m i\u00e7in bir model geli\u015ftirmektedir.  Bu modelin odak noktas\u0131 belirli yollar veya ko\u015fullar de\u011fil, sistemimizdeki en karma\u015f\u0131k hastalar\u0131 nas\u0131l daha iyi destekleyebilece\u011fimizdir ve bunu ele alabilirsek, faydalar\u0131n t\u00fcm n\u00fcfusumuzda g\u00f6r\u00fclece\u011fini kabul etmektedir.
