About us

The North and West Bristol Locality Partnership is one of six locality partnerships across Bristol, North Somerset and South Gloucestershire, serving 183,700 people across 14 GP practices. Often broken down into inner wards, areas such as Clifton and Henleaze and outer wards, areas such as Horfield and Southmead.

We bring together residents, local community groups and organisations, GPs, hospitals and care services to work more closely in supporting people.

We want to help build happy, healthy communities. We believe being well means more than just not being ill. It’s about feeling welcome, having friends and support, and enjoying things like good food, green spaces and safe homes.

Our partnership is about joining things up, not replacing what’s already there. We want people’s experience of health and care to feel simple and make sense. By working together, listening to our communities, and celebrating our differences, we hope to make life better, fairer and healthier for everyone in our area.

Our priorities

Our key priorities are to:

  • Improve children’s mental health and reduce anxiety by supporting young people moving from Year 6 to Year 7 who are at risk of emotionally-based school avoidance
  • Explore ways to support healthy weight in Year 6 children
  • Decrease incidences of deliberate self harm in 10-24 year olds
  • Explore the impact of alcohol and substance misuse on people
  • Promote healthy ageing through more accessible social prescribing and changes in the way voluntary sector and health services work together
  • Improve the wellbeing and health of people living with Chronic Obstructive Pulmonary Disease (COPD), and their families, including increasing access to pulmonary rehabilitation in our outer areas
  • Improve the wellbeing and health of people with diabetes, and their families
  • Develop a Mental health and wellbeing Integrated Network Team (MINT) offering psycho-social support
  • Consider how MINT can support people with dementia
  • Contribute to a Bristol-wide approach to reducing falls.
