North and West Bristol Locality Partnership is made up of partners from across health and social care along with the voluntary sector and other groups that offer support to local communities.

We work with residents, local community groups and organisations, GPs, hospitals, care services and other partners. We bring the right people together at the right time to listen, problem solve or co-design services. 

Our partners include: 

• Affinity Primary Care Network – a collaboration of GP practices working together to provide healthcare services
• Age UK Bristol – information and advice for people aged over 55 living in Bristol
• Avonmouth Community Centre – runs a friendly and accessible hub for people living in Avonmouth and the surrounding area
• Avon and Wiltshire Mental Health Partnership NHS Trust – mental health services provider
• Bristol City Council – the local authority for Bristol
• Bristol NHS Group – a partnership between North Bristol NHS Trust and University Hospitals Bristol and Weston NHS Foundation Trust
• Caafi Health – helps communities get the health and care services they need
• Care & Support West – provides independent information on social care in the West of England
• Carers Support Bristol and South Gloucestershire – provides support, information and advice to carers of any age living in Bristol and South Gloucestershire
• Centre for Sustainable Energy – gives information and advice on sustainable energy
• Community Pharmacy Avon & Wiltshire – represents community pharmacies in the area
• Family Hubs (Bristol City Council) – a hub that provides support for children, young people and families, offering services including parenting advice, early years activities and access to health and wellbeing resources
• Healthwest Primary Care Network – a collaboration of GP practices working together to provide healthcare services
• Lockleaze Neighbourhood Trust – provides community-led activities and projects for the residents of Lockleaze
• Northern Arc Primary Care Network – a collaboration of GP practices working together to provide healthcare services
• North Bristol Advice Centre – provides free and independent welfare benefits and debt advice for Bristol and South Gloucestershire
• One Care – represents and supports GP practices across Bristol, North Somerset and South Gloucestershire
• Phoenix Primary Care Network – a collaboration of GP practices working together to provide healthcare services
• Sirona care & health – a community health services provider
• Southmead Development Trust – works to improve health, wellbeing and employment within the local community.
