The Health and Happiness Hubs support patients with long-term health conditions to receive holistic help and support through an innovative partnership between GP practices and Voluntary, Community and Social Enterprise (VCSE) organisations.
This partnership is underpinned by population health management, responding to the needs of our local population in a way that is accessible to local communities.
Find out more about Health and Happiness HubsThe WARM service provides confidential, accredited and independent debt, benefits and welfare advice for those accessing primary care and VCSE health and wellbeing services. This is often as a result of socio-economic issues and financial hardship which are impacting on an individual’s mental health.
The service is run by Citizens Advice South Gloucestershire and funded by Bristol, North Somerset and South Gloucestershire Integrated Care Board.

This initiative aims to improve physical activity, motivation, and emotional wellbeing for individuals referred into the service, with a focus on sustaining long-term benefits.
It also seeks to identify service gaps that could enhance the Work Well West programme and address health inequalities by increasing accessibility for underserved groups, such as those with early onset Type 2 Diabetes, chronic pain, mental health conditions and carers.
The South Gloucestershire Village Agent Plus service takes a place-based and integrated care approach to help reduce social isolation and loneliness in rural areas, ensuring individuals remain independent in their home and community for as long as possible.
