Personalised Medication Support Service

The Personalised Medication Support Service is a trial project designed for anyone who finds it difficult to take medication on a regular basis. This can be for yourself, a family member, or someone you are caring for.

Diabetes or high blood pressure can develop in older age and it’s important to take medications regularly to help manage these conditions. However, this can be difficult when you’re taking many different medications and have problems remembering what to take and when.

The Personalised Medication Support Service can help by providing support through the use of electronic reminders. These could be simple text messages on your phone, or internet enabled video or audio base reminders. Our community project workers are trained to assess your needs and recommend what system might be best for you.

This service is for those who:

  • live in Bristol, North Somerset or South Gloucestershire
  • take multiple medications
  • struggle to remember doses
  • live with a chronic condition e.g dementia, diabetes, or high blood pressure
  • support someone who has difficulty managing their medication.

What resources are available?

If you, or someone you are caring for is having difficulty remembering to take their medication on a regular basis, there are resources available to help.

SMS messaging reminders

These reminders are set up through your local pharmacy and sent straight to your mobile phone. You can get up to 4 messages a day. ​You will need a mobile phone for this service.

Medisafe Pill Reminder App

Medisafe is free Smartphone app which can be set up track and manage your medications, including setting reminders and logging doses.

It also has a supporter account function which allows family members to help you track your medication.

YourMeds Dosette Box

The YourMeds Dosette Box is a digital box is filled with your medicines. It gives visual and audio alerts to show you which compartment to open and when and will send an alert to a carer or family member if your dose is missed.

My Home Helper

My HomeHelper is a simple device that makes it easy for family members or carers to stay in touch for check-ins and medication reminders.

Sentai AI companion

The Sentai AI companion smart speaker provides support with your daily routine, by providing audio reminders and messages. It is designed to blend into the home environment

MemRabel 2

MemRabel 2 provides is an audio-visual dementia clock that supports you with reminders for daily tasks, medications or appointments which play back at scheduled times. MemRabel 2 does not require an internet connection.

HomeSight

HomeSight allows video calls from your family member or carer through your television so they can provide you with assistance when taking your medications.

Other types of support

If you, or someone you care for, are finding it difficult to take medication due to swallowing problems, reduced dexterity, or poor eyesight, you should speak to your GP or local pharmacist. They can provide advice and help you find a solution that works best for your needs. This might include options such as:

  • Large-print labels

  • Pill splitters

  • Soluble or liquid versions of medication.
