Refreshed Joint Forward Plan webpages published
The Joint Forward Plan for the Healthier Together health and social care system has been refreshed to reflect our priorities for 2025 to 2030.
Our Joint Forward Plan was first published in June 2023 and is updated annually to take account of the progress being made, new evidence, and to reflect NHS planning guidance.
This updated version continues to set out how our Integrated Care System (ICS) will:
- Deliver improvements in population health and wellbeing ambitions
- Reduce health inequalities
- Deliver high-quality services informed by system-wide intelligence
- Improve the efficiency and sustainability of services.
The plan continues to focus on key areas including support for children and young people, mental health, learning disabilities and autism, primary and community care, and acute services.
You can find an updated summary of our Joint Forward Plan in a dedicated section of the Healthier Together website, along with the full document available for download.
This summary includes a new section on trauma-informed approaches, which recognises the widespread impact of trauma and the importance of creating services that are compassionate and more equitable.
