Personalised Medication Support Service

The Personalised Medication Support Service is for anyone who finds it difficult to take medication on a regular basis. This can be for yourself, a family member, or someone you are caring for.

As people age, they can develop conditions like diabetes or high blood pressure. It’s important to take medications regularly to manage these conditions.

The Personalised Medication Support Service is looking at the use of electronic reminder devices to see how effective and reliable they are in helping people remember to take their medications.

How does the service work?

A Personalised Medication Support project worker will arrange a visit with you to discuss how you manage your medication and give advice. They may also recommend that you try an electronic medication aid.

If you would like to try the recommended electronic medication aid, you’ll be offered a free three-month loan of the device, which includes help with setup and regular check-ins with your project worker. We may also recommend that a family member or carer help you with using the device.

Some of the devices we can provide include:

  • Smartphone reminder app
  • SMS messaging service
  • Smart Dossette Box Device

You may need a mobile phone, smart phone or internet at home to use the device. If none of these options are suitable, we can give you further advice and support.

After the three-month trial, your project worker will ask for your feedback on how you found the device and can provide options for you to continue using it. All your personal details will be kept confidential.

Phase 2 of this project is due to finish at the end of September 2024. However, if you are interested please contact the team for information regarding Phase 3 of the project, due to start at the end of 2024.
