South Gloucestershire Locality Partnership is made up of partners from across health and social care along with the voluntary sector and other groups that offer support to communities locally. These include: 

• Farmacia comunitară Avon & Wiltshire - reprezintă farmaciile comunitare din zonă
• Concorde Mendip Vale Primary Care Network – a collaboration of local GP practices working together to provide healthcare services
• Healthwatch South Gloucestershire – an independent organisation that gathers the public’s views on health and social care services to help improve them
• Network 4 Primary Care Network – four GP practices (Downend Health Group, Green Valleys Health, The Orchard Medical Centre and Three Shires Medical Practice) working together to provide healthcare services
• Trustul NHS din Bristolul de Nord – provides hospital and community healthcare to the residents of Bristol, North Somerset and South Gloucestershire
• OneCare - reprezintă și sprijină practicile medicilor de familie din Bristol, North Somerset și South Gloucestershire
• Consiliul Gloucestershire de Sud – the local authority for South Gloucestershire
• Îngrijire și sănătate Sirona – community health services provider
• Severnvale Primary Care Network – five GP practices (Almondsbury Surgery, Pilning Surgery, Severn View Family Practice, Streamside Surgery and St Mary Street Surgery) working together to provide healthcare services
• Southern Brooks –  a charity organisation that provides a range of community development, health, wellbeing and support services
• Stokes Medical Group – a collaboration of local GP practices working together to provide healthcare services
• Grupul Vita Health – offers Talking Therapies to adults aged 16 and above who are suffering from  conditions including depression, anxiety and excessive worry
• West of England Rural Network – supports sustainable, resilient and inclusive rural communities
• Yate and Frampton Primary Care Network – a collaboration of local GP practices working together to provide healthcare services.
• 4PCN – four GP practices (Hanham Health, Kingswood Health Centre, Cadbury Heath Healthcare and Close Farm Surgery) working together to provide healthcare services.
