Personalised Medication Support Service
The Personalised Medication Support Service is a trial project designed for anyone who finds it difficult to take medication on a regular basis. This can be for yourself, a family member, or someone you are caring for.
Diabetes or high blood pressure can develop in older age and it’s important to take medications regularly to help manage these conditions. However, this can be difficult when you’re taking many different medications and have problems remembering what to take and when.
The Personalised Medication Support Service can help by providing support through the use of electronic reminders. These could be simple text messages on your phone, or internet enabled video or audio base reminders. Our community project workers are trained to assess your needs and recommend what system might be best for you.
This service is for those who:
- live in Bristol, North Somerset or South Gloucestershire
- take multiple medications
- struggle to remember doses
- live with a chronic condition e.g dementia, diabetes, or high blood pressure
- support someone who has difficulty managing their medication.
